Sports physicals for groups or teams




We offer groups and teams discounts for sports physicals, subject to the following requirements:  
  • Team or group of three or more
  • An appointment made at least one day in advance outside of our walk in hours
  • All athletes must be accompanied by at least one responsible adult, who must assist our staff.  Larger groups will require more adult supervision.
  • All forms completed and turned in on arrival
  • Athletes must not require immunizations, screenings or related paperwork.
  • Payment in full upon arrival ~ we will provide a receipt for the group but we will not do any insurance company paperwork or provide individual receipts ~ we accept cash or a single credit card transaction
In exchange for meeting these requirements we will charge $10 per person instead of our usual fee of $30

We will be glad to serve athletes needs who can't meet these requirements following our normal terms and conditions.  Thank you for your patronage.
