There are changes to "Terms of Service" effective July 1, 2012
The Children’s Clinic at St. Lawrence and St. Lawrence Pediatrics are a faith based partnership of Patricia L. Dyer, PNP and St. Lawrence Church. We are located in the Franciscan Guest House behind St. Lawrence Church at 1520 East Delavan Avenue, Buffalo, NY 14215. Currently, we receive no government funding except Medicaid fees for service and managed care. The church provides office space and utilities to reduce our operating expenses. In exchange, our fees are substantially reduced. Most of our services are limited to people under age 21 - vaccines are usually limited to those under 19. Consistent with our beliefs we do not discriminate based on race, color, sex, national origin, disability, religion or sexual orientation.
We also do not turn away anyone without insurance or unable to pay. We do require patients with insurance in force to provide us with information and an authorization to bill their insurance company. If patients have insurance in force that will not pay us, either because the insurance company will not, or the insured party will not authorize it, we do require payment based on the schedule below. To receive free services you may be required to complete an application.

Basic Office Visit $30.00 each
This includes sick child visits and physical exams. Two copies of the NY State standard physical exam form and completion of any form presented at the time of the exam are included.
Expanded Office Visit $90.00 per hour
You can hire us on an hourly basis for help with complex issues, multiple referrals etc. Please feel free to ask for details.
Vaccine administration $5.00 each shot
This includes updating the NY State Immunization Information System and two printed copies of the shot record.
Vaccines cost
Many patients will qualify for the Vaccines for Children Program. In general, children 18 years old and under without commercial insurance will likely qualify, along with many Medicaid patients. If so, the program pays for the vaccines. If not, we will inform you of availability, delivery time and the cost and collect it before ordering the vaccine. We do not stock non-program vaccines because they are perishable with short shelf lives and high and variable costs.
Document preparation $5.00
This fee is for updating your child’s NYSIIS record with shots obtained elsewhere and for providing documents after service. If you need medical records printed or updated we will help as much as we can. This fee helps to pay for the paper, ink, computers, phone bills and internet access required. If your child is enrolled in our EHR (children under 10) you have free access to records from home at all times! Have e-mail? Ask us to enroll your child.
Lab tests are provided based on cost of materials.
Blood Lead Analysis $10.00
Hematocrit included in cost of physical exam
Tuberculosis [TB] test $10.00
Important: If you have no insurance and no money we will provide care. You may be required to complete an application, and we ask that you pay what you can afford today and remember to help us later when things get better financially. We do not bill and operate on the honor system. Please remember that the money we collect goes to support our ability to provide care for those in need.We accept cash, MasterCard, Visa, Discover, American Express and PayPal. We do reserve the right to revise our fee schedule based on current conditions without notice.
